Fall Soccer Season Late Registration

1. Go to www.eayso.org and "pre-register" your child. Once you have done so, print out 2 copies of the medical release form (not the parent volunteer form) per player. PLEASE MAKE SURE THAT YOU SIGN AND DATE THE FORMS. Make sure you initial the second page.
2. Send the signed forms along with a check for $85.00 per player, plus a $10 Late Registration Charge, to the PHMSA Registrar at the following address:

   NOTE: If your child has not played PHMSA before, please include a copy of their birth certificate for age verification. For their protection we will shred the copy once we have verified their age.


3. Once your properly competed forms are received, you will be placed on our waiting list. The earlier that we receive your forms, the more likely your child will be placed on a team without delay.
4. We do our best to place every player. If we cannot place your child we will return your forms and check shortly after the season starts in September.
5. Remember that PHMSA is an all-volunteer organization. We need Referees and Coaches, so if you can help us, please let us know by contacting our Coach
